This form should be used by anyone who has been granted a permit to work to undertake work near our infrastructure.
The aim of the form is to enable onsite workers to
- Call in before work starts
- Call out before leaving the site
In addition to reporting every day work is undertaken onsite - you must notify us when all works are finished by completing the closure form.
